NamespaceManager produces white pages

Creation of Category pages, created flag "NamespaceManager Required!"
Addition of NamespaceManager Extension returns blank white pages; removal of require_once statement restores site with warning still intact.
I have followed the instructions carefully, multiple times, and I just do not get it .. what do you think?
