Trouble Using ReaderFeedback Extension with GuMaxDD Skin

Hi, I installed the ReaderFeedback extension on my wiki (http://www.mediawiki.org/wiki/Extension:ReaderFeedback)
This extension displays a review form at the bottom of each page.
The review form is not visible when using the GuMaxDD skins, but is visible in other skins like Vector.
Appreciate any help to fix this problem. Thanks!
